NG+ all the way. I played through Witcher like 3-4 times, but only once from start. You get to chose your gear and if it's the Witcher grandmaster you can use it for a looong time, especially with runewords. The standard new equipment looks good, but doesn't have any decent stats.
I wanted to change my playstyle and get the urisine armor for new game plus. But I didnt think I would have to farm it in the base game for me to upgrade it in NG+. This means I have to wait til I get to toussiant to change to any armor. I only farmed the griffin and wolf.

But like pablofreak88 said if you never saved over you regular ng save that you used for ng plus. You can start ngplus over again using the regular new game save. As long as you don't save over it or delete it. As you enter the ruins, turn right and go down to the underground level. At the end of the narrow tunnel, you will have two more level 21 Alghouls to defeat. The chest with the Enhanced Ursine silver sword diagram is behind them. I recently finished the core game at death march, and was debating either starting a completely new game, or a new game+. I finished the game at around lvl 37, and I was wondering what would actually be a "harder" play. My experience was that the game was often quite challenging on death march up HzYn.
